Residential Services facilitate independence and promote integration into the community. The supports include 24 hour supervision and training in activities such as: meal prep, laundry, household care and maintenance, activities of daily living such as: bathing, eating, dressing, personal hygiene, shopping and money management. Residential Supports include socialization, relationship building, leisure choices and participation in generic community activities.
Day Training Services
Day Training services support and encourage the participant to take part in daily meaningful routines in the community. Day training services stress training in the activities of daily living, self-advocacy, adaptive and social skills and vocations skills. Day training services are designed to foster the participant’s acquisition of skills, build positive social behavior and interpersonal competence, foster greater independence and personal choice. Day training services are person centered and designed to support employment related goals.
Community Living Supports
Community Living Supports facilitate independence and promote integration into the community for participant’s residing in their own home. The supports include assistance, support and training in activities such as: meal prep, laundry, household care and maintenance, activities of daily living such as: bathing, eating, dressing, personal hygiene, shopping and use of money, reminding, observing, monitoring of medications, and non-medical care (not requiring nurse or physician intervention).
Supported employment is intensive, ongoing support for a participant to maintain paid employment in an environment in which an individual without a disability is employed.
Respite services are services provided to individuals unable to care for themselves; furnished on a short-term basis because of the absence or need for relief of those persons normally providing the care. Respite is available only to individuals living in family homes or their own homes.
Case Management Services
Case Management involves working with the participant and others that are identified by the participant in the development of a Person Centered Plan of Care (POC). Case management is responsible for the assessment, reassessment, appropriate evaluations, intake, referral, and eligibility processes as well as the ongoing monitoring of service delivery. The case manager shall ensure the following principles: empowerment, community inclusion, health and safety assurances, and use of formal, natural and community supports.
Michelle P. Waiver
The Michelle P. Waiver (MPW) is a home- and community-based waiver program within the Kentucky Medicaid program developed as an alternative to institutional care for individuals with intellectual or developmental disabilities. MPW allows individuals to remain in their homes with services and supports.
